If you have insurance through Zurich’s superannuation fund, use this form to nominate which of your dependents receive your benefits in the event you pass away.
Decline indexation increases
The inflation protection benefit protects the value of your insurance against the impact of inflation by increasing your insurance cover automatically each year. If you don't want this to happen, this form allows you to reject offers to increase your premiums.
Rollover Authority
If you have insurance through Zurich's Superannuation plan, use this form to pay premiums using funds from another superannuation fund.
Change of financial adviser
Use this form to change your financial adviser on your Zurich policy.
Waiting period reduction feature
